During an FDA inspection of a registrational bioequivalence study, an issue was discovered when the inspector reviewed the site’s freezer maintenance logs, which indicated a past temperature malfunction. The lab manager could not provide documentation on the chain of custody of the critical pharmacokinetic samples, including freezer inventory logs and if the samples were moved during the temperature excursion. As a result, the site received a Form 483 from the FDA.
